Case Management services provide clients with information, referrals and coordination of resources including long term care placement, respite services, medical & mental health needs, in-home assistance, and caregiver support. The social worker assesses the client system, sets goals with the client, and develops a person-centered service plan to achieve goals. Direct services may include food, transportation, vocational, housing, and utility assistance. JFS administers allocations of Keren Ami funds held by the WNC Jewish Federation to Jewish households.
Clients who seek help from JFS are experiencing crisis situations or ongoing serious life challenges affecting their ability to function successfully in daily life. Clients receive individualized attention, including home visits, accompaniment to appointments, and advocacy for other agency and government assistance. These clients and their families learn strategies for problem-solving, coping capacities, and accessing resources. As a result, they experience improved ability to function successfully, and enhanced well-being and quality of life. The JFS approach to case management is person-centered, and includes seeing the whole client system, including environment, family, health, spirituality and community.
The JFS staff works in close collaboration with area synagogues and Rabbis to ensure safe, effective and appropriate case assistance to individuals who seek help through that entry point.
